      Heart failure is a condition that is increasing in prevalence in the UK, with high patient mortality rates and frequent hospital admissions. Nurse-led heart failure services help to improve patient outcomes through aiding pharmacological, interventional and holistic care. Heart failure management teams that work to support patients can vary greatly from service to service.
