      This article reports that along with the results from its 2000 Medication Safety Self-Assessment, the Institute for Safe Medication Practices (ISMP) introduced an updated version during a presentation at the sixth Annual Patient Safety Congress in Boston, Massachusetts. The new publication was to be mailed to participating hospitals in May 2004. The assessment queried participants on ten key elements within the medication distribution system. The ISMP found the greatest opportunities for improvement laid in the key element areas of patient information, drug information, communication of drug orders, patient education, and quality and risk management.
